Why Senior Care Management Solutions?
SCMS provides collaborative services to promote continuity of care. From Professional Care Management and Nursing Care to Personal Care Services, our team of professionals work together to ensure the highest standard of care. SCMS standard of care is unparalleled to other organizations because of the quality of individuals employed. SCMS provides care management services under the stringent guidelines set forth by the Aging Life Care Association®. Care is provided by Registered Nurses (RN) and Certified Nursing Assistants (CNA)/Licensed Nursing Assistants (LNA) who are regulated by the State Board of Nursing. SCMS never uses unskilled, unqualified personnel because your loved one deserves only the best.
The owners
Melissa Johnson, RN, MHA, CHPN, CCM
After serving four years active duty in the U.S. Army, Melissa moved back to Arizona to attend nursing school. Melissa graduated from Arizona State University with a Bachelor's Degree in the Science of Nursing in 2005. Melissa continued her education by receiving her Master's Degree in Health Administration from University of Phoenix in 2009. During her time as a nurse, Melissa cared for the elderly and supported their family throughout the continuum of care to include Intensive Care, Home Health, Private Duty and Hospice and Palliative Care. Melissa is passionate in assisting seniors to have a quality of life, while aging in the comfort of their own environment. Melissa is also experienced in working with families to ensure the safest environment, highest quality of care and advisement through disease processes. Melissa specializes in Alzheimer's disease management and End of Life care.
Melissa is an advocate and volunteer for the Alzheimer's Association, providing community education on Alzheimer's disease and related Dementia's. Melissa is a member of the Aging Life Care Association, National Hospice and Palliative Care Organization and National League for Nursing. Melissa holds a Certification in Case Management, as well as a Certification in Hospice and Palliative Nursing.

Sarah Brambila, MSW
Sarah Brambila is a seasoned Social Worker with a wealth of experience. She moved to Arizona in 2005 after completing her Bachelor’s Degree in Psychology from University of Nebraska-Lincoln. Sarah started her career working for a local non profit that provided brief intervention therapy to families involved with Department of Child and Family Safety. After 7 years, she decided to pursue her Master’s Degree in Social Work to expand her skills in empowering families, counseling, and connecting her clients to needed resources and support systems. Sarah graduated from Arizona State University in 2013 with her MSW and spent a two years working in youth substance abuse prevention providing community education presentations, facilitating a coalition with resident and community stakeholders, and forged new partnerships with other community groups to expand the efforts.
When the state funding depleted, Sarah joined the Muscular Dystrophy Association (MDA) as an Associate Director of Care and Clinical Services. She supervised care teams in Arizona, Nevada, New Mexico, El Paso, Utah, Southern California and Hawaii to ensure all families impacted by neuromuscular disease had access to MDA Care Center multidisciplinary teams as well as a wealth of resources and other supports. From there an opportunity arose with one of the MDA’s most trusted team members, BAYADA Home Health Care. Sarah was offered a position as Director of a Phoenix BAYADA Personal Care office where she worked tirelessly to rejuvenate the office, balance the budget and also to ensure that services were delivered with Compassion, Excellence and Reliability.
After years of building a strong team of professionals and a forging through the uncertainty of COVID and a global pandemic, Sarah has decided to return to her Care roots. Sarah is honored and privileged to join the efforts of the Senior Care Management Solutions team as their newest Care Manager.

2021
A second Care Manager, Sarah Brambila, joined the SCMS care management team. Sarah is in the process of obtaining a Commission for Case Manager Certification (CCMC) and is our first Social Worker. In addition to Sarah joining the team SCMS retained all employees and added another 5 CNA/LNA’s.

2020
Was a difficult year for everyone. SCMS was able to retain all employees, implement successful COVID-19 protocols and add 4 new CNA/LNA’s to our team.

2019
A second Assistant Care Manager, Mawa Jabeteh, joined the care management team. Mawa has worked for SCMS in the capacity of a Licensed Nursing Assistant, as well as taking courses to obtain a nursing degree. Mawa has shown great compassion and expertise in her field. Mawa is in the process of obtaining a Certification in Senior Advisement. Mawa will also be responsible for staff engagement and training.

2018
In 2016, the Arizona State Board of Nursing proposed a plan to create two different categories of nursing assistants. When the law was finally created, the level of oversight for a Certified Nursing Assistant returned to the previous minimal federal level, which does not require a criminal background check. The new distinction, Licensed Nursing Assistant required a criminal background check, as well as increased oversight from the State Board of Nursing. The official change took place in 2018. In SCMS's dedication to provide the highest quality of service, Certified Nursing Assistants began the process to change to a Licensed Nursing Assistant.

2017
SCMS continued to experience growth throughout the company adding 5 new certifed nursing assistants (CNA). In addition to adding new CNA's, SCMS started offering placement services to current clients.

2016
SCMS continued to experience growth throughout the company. Lisa Dunbar, Assistant Care Manager, joined the SCMS team to provide care management services as well as nursing services. Lisa is a Registered Nurse with 30+ years of experience.

2015
National Association of Professional Geriatric Care Managers changed their name to Aging Life Care Association to reflect the wider range of expertise a care manager encompasses. An Aging Life Care Expert is required to demonstrate stringent education, experience and certification requirements to be a member of the organization. Melissa Johnson is an Advanced Professional, which is the highest distinction with the Aging Life Care Association.

2014
SCMS realized the value in providing personal care assistance to compliment Geriatric Care Management (GCM) services and began providing private duty care. SCMS is capable of providing high level nursing care in the comfort of the client’s home as well as a variety of personal care services by a Certified Nursing Assistant (CNA). SCMS only offers professional, experienced staff that are regulated by the AZ State Board of Nursing.

2013
SCMS is passionate about serving those in the community affected by dementia through expert care and education. Owner, Melissa Johnson, began volunteering for the Alzheimer’s Association, providing education to the community on dementia.

2012
Owner, Melissa Johnson, obtained her certification in case management and became and active member of the NAPGCM. Membership to the NAPGCM is reserved for healthcare professionals that meet specific requirements to demonstrate expertise and professionalism in the field of Geriatric Case Management. Melissa also renewed her certification in Hospice and Palliative Nursing (CHPN) and has been dedicated to serving those with a terminal illness for nearly a decade.
